Minnesota Immunization Requirements
 
 
Minnesota Statutes, Section 121A.15, requires all students enrolling in a public elementary or secondary school to show evidence of immunization or documented exemption.

I require a print out from your health care provider as proof of immunizations. No parent phone calls or handwritten notes will be accepted.  This can be a print out received at the clinic, a printout from your child's medical portal, or a printout of their immunization record you access through the Docket App.

If your child will be entering Kindergarten, 7th or 12th grade in the fall, please complete the required immunizations and turn in the proper documentation as soon as possible, as additional immunizations are required for these grades.

Students who cannot be vaccinated due to a medical condition, or whose parents are conscientiously opposed to immunizations, may receive a legal exemption. The form to register this exception can be obtained under "Health Office Forms". The form (along with notarization or physician signature) needs to be submitted to the school prior to the child’s first day of school.
